Dr Valerie Teo Hui Ying
Family Physician, Senior Consultant; Head, Kallang Polyclinic, National Healthcare Group Polyclinics

Valerie is a Family Physician, Senior Consultant and Head of the Kallang Polyclinic (NHGP) and Adjunct Assistant Professor, Lee Kong Chian School of Medicine. She has special interest in social-health integration, respiratory medicine, medication use and safety as well as innovation and technology. She is the clinical program lead for the RELATE (RELationship-based heAlth and social inTEgration)  and ENRICH (Engaging and empowering out Community for better Health) programmes in NHGP which proactively identifies the unique needs of complex patients to provide them with a shared care plan that the healthcare team together with community partners formulate with and for the patient’s care and also helps to link patients up with community providers for various programs such as exercise, socialisation and mental wellness. The programme has allowed close collaboration with multiple community partners and has strengthened the network to support patients. Valerie also champions telehealth initiatives and innovation in primary care, this includes successful programs such as primary tech enhanced care – hypertension (PTEC) which leverages on technology to empower patients to self-manage through a home BP monitoring device, mobile application and chatbot that provides timely advise and feedback.
